Job summary

Legacy Hospice is seeking a compassionate Visit Registered Nurse VRN to join our dedicated team in home hospice care. In this role youll provide vital care and support to patients in their homes, supplementing our RN Case Managers and ensuring timely responses to urgent needs.

Youll perform nursing assessments, develop care plans, administer medications, educate families, and collaborate with the interdisciplinary team to maintain high-quality hospice care.

Qualifications

• Active RN license in the state of employment.
• CHPN and/or BSN preferred; hospice experience preferred.
• Minimum 1 year hospice nursing experience; 2 years preferred.
• Active BLS certification.
• Valid driver's license and reliable vehicle.

Responsibilities

• Perform focused nursing assessments and address acute needs of hospice patients.
• Ensure continuity of care through communication with the interdisciplinary team.
• Complete initial and comprehensive assessments of patients and families to determine hospice needs.
• Develop and revise individualized care plans in collaboration with the patient and caregiver.
• Administer prescribed medications and treatments and educate families on care techniques.
• Counsel patients and families on symptom management and end of life issues.
• Maintain accurate and timely clinical documentation.
• Assist patients with activities of daily living and promote comfort focused care.
• Respond promptly to changes in patient condition or caregiver concerns.
• Collaborate with case managers to ensure needed supplies and services are provided.
• Participate with interdisciplinary team meetings and quality improvement initiatives.
