Legacy Hospice is seeking a compassionate Visit Registered Nurse (VRN) to provide in-home hospice care and support to patients. You will perform nursing assessments, develop care plans with patients and families, administer medications, and educate families on care techniques.

The role emphasizes symptom management, end-of-life care, and timely, accurate documentation while collaborating with case managers to ensure needed supplies and services are available.

Responsibilities

• Perform focused nursing assessments and address acute needs of hospice patients as directed
• Ensure continuity of care through effective communication with the interdisciplinary team
• Complete initial and comprehensive assessments of patients and families to determine hospice needs
• Develop and revise individualized care plans in collaboration with the patient and caregiver
• Administer prescribed medications and treatments, and educate families on care techniques
• Counsel patients and families on symptom management and end-of-life issues
• Maintain accurate and timely clinical documentation
• Assist patients with activities of daily living and promote comfort-focused care
• Respond promptly to changes in patient condition or caregiver concerns
• Collaborate with case managers to ensure needed supplies and services are provided
