Job summary

Legacy Hospice is seeking a compassionate Visit Registered Nurse (VRN) to provide in-home hospice care in Cape Girardeau, MO. You will support RN Case Managers, respond to urgent needs, and ensure timely, quality patient care in a home setting.

Ideal candidates are licensed RNs with BLS, hospice experience, and CHPN/BSN preferred. This PRN, hourly role offers autonomy, mileage reimbursement, and opportunities for professional growth within a mission-driven team.

Qualifications

• Registered Nurse (RN) licensed in the state of employment.
• Certified Hospice and Palliative Nurse (CHPN) and/or BSN preferred.
• Minimum 1 year of hospice nursing experience required (2+ years preferred).
• Active Basic Life Support (BLS) certification.
• Strong written and verbal communication skills.
• Self-motivated and capable of working with minimal supervision.
• Valid driver’s license, reliable vehicle, and proof of insurance.
• Legacy Hospice is a drug-free workplace. Pre-employment drug screening is required, including testing for marijuana.

Responsibilities

• Perform focused nursing assessments and address acute needs of hospice patients as directed.
• Ensure continuity of care through effective communication with the interdisciplinary team.
• Complete initial and comprehensive assessments of patients and families to determine hospice needs.
• Develop and revise individualized care plans in collaboration with the patient and caregiver.
• Administer prescribed medications and treatments, and educate families on care techniques.
• Counsel patients and families on symptom management and end-of-life issues.
• Maintain accurate and timely clinical documentation.
• Assist patients with activities of daily living and promote comfort-focused care.
• Respond promptly to changes in patient condition or caregiver concerns.
• Collaborate with case managers to ensure needed supplies and services are provided.
• Participate in interdisciplinary team meetings and quality improvement initiatives.
