Job summary

Legacy Hospice, Inc. is seeking a compassionate Visit Registered Nurse (VRN) to join our home-based hospice team in Demopolis, AL. This PRN, hourly role supports RN Case Managers and ensures timely responses to urgent patient needs.

Responsibilities include patient assessments, care plan development, medication administration, symptom management education, and thorough documentation. Must have RN license in Alabama, active BLS, and at least 1 year hospice experience; CHPN/BSN preferred.

Qualifications

• Must hold RN license in the state of employment.
• CHPN certification and/or BSN preferred.
• Minimum 1 year hospice nursing experience; 2+ years preferred.
• Active BLS certification required.
• Strong written and verbal communication skills.
• Valid driver’s license with reliable vehicle and proof of insurance.

Responsibilities

• Perform focused nursing assessments and address acute needs of hospice patients.
• Ensure continuity of care through communication with the interdisciplinary team.
• Complete initial and comprehensive patient assessments to determine hospice needs.
• Develop and revise individualized care plans with patient and caregiver.
• Administer prescribed medications and educate families on care techniques.
• Counsel patients and families on symptom management and end-of-life issues.
• Maintain accurate and timely clinical documentation.
• Assist patients with activities of daily living and promote comfort-focused care.
• Respond promptly to changes in patient condition or caregiver concerns.
• Collaborate with case managers to ensure needed supplies and services.
• Participate in interdisciplinary team meetings and quality improvement initiatives.

Legacy Hospice is seeking a compassionate and experienced Visit Registered Nurse (VRN) to join our dedicated team. In this role, you’ll provide vital care and support to patients in their homes, supplementing our RN Case Managers and ensuring timely responses to urgent needs.

Qualifications

• Registered Nurse (RN) licensed in the state of employment
• Certified Hospice and Palliative Nurse (CHPN) and/or BSN preferred
• Minimum 1 year of hospice nursing experience required (2+ years preferred)
• Active Basic Life Support (BLS) certification
• Strong written and verbal communication skills
• Self-motivated and capable of working with minimal supervision
• Valid driver's license, reliable vehicle, and proof of insurance
