Job summary

Legacy Hospice, Inc. in Cape Girardeau, MO is seeking a compassionate Registered Nurse Case Manager (RNCM) to deliver high-quality end-of-life care in patients' homes and to support families with empathy and respect.

This full-time, salaried exempt role involves coordinating care with the interdisciplinary team and providing clinical expertise in a home-based setting. Requirements include an active RN license in the employing state, CPR/BLS certification, and 1 year of hospice/home health

Qualifications

• RN license in state of employment
• BSN or CHPN preferred
• Minimum 1 year of nursing experience in hospice, home health, or related field
• Strong interpersonal, clinical and organizational skills
• Active CPR/BLS certification
• Valid driver's license, auto insurance and reliable transportation

Responsibilities

• Perform comprehensive patient assessments and create individualized care plans
• Manage a caseload of hospice patients coordinating care with the interdisciplinary team
• Administer medications and treatments per physician orders
• Educate patients and families on symptom management, medication and end-of-life care
• Monitor and update care plans based on ongoing patient assessments
• Provide emotional and clinical support to patients and caregivers
• Communicate with physicians, care partners and community resources
• Maintain accurate, timely clinical documentation
• Participate in interdisciplinary team (IDT) meetings and on-call rotation
• Supervise ancillary staff and ensure continuity of care
