Virtual Nurse Navigator (Remote) 10-7p EST, M-F

The Virtual Nurse Navigator will utilize their professional nursing skills, clinical experience, empathy, and relationship-building abilities to provide personalized virtual support to patients and healthcare professionals (HCPs). The role focuses on reimbursement, patient access, and therapy coordination, ensuring seamless treatment access. The Navigator will assist HCPs in overcoming access barriers and collaborate with vendors, pharmacies, and field teams to optimize patient care. The role combines clinical knowledge with technical skills to deliver exceptional support, service, and guidance.

Essential Functions:

  • Provide virtual omnichannel support via email, chat, text, virtual connections, and calls.
  • Engage patients and HCPs with empathy and emotional intelligence, building trust and rapport.
  • Support HCP office issues related to REMS, reimbursement, insurance, and dispensing.
  • Act as a primary contact for patients, offering disease/therapy education and access assistance (benefit verification, prior authorizations, insurance navigation).
  • Collaborate with the Momentum Inbound team to identify gaps and improve processes.
  • Communicate complex information clearly and empathetically.
  • Complete patient engagements accurately based on criteria.
  • Contribute to program success through partnerships with patients, field, and operational teams.
  • Work cohesively within the team to identify opportunities and escalate issues.
  • Provide feedback to managers and teams for product and support improvements.
  • Ensure compliance with all regulations and complete required reporting.

Required Education and/or Experience:

  • Experience in patient access, reimbursement, or care coordination support roles.
  • Experience in high-volume inbound/outbound call centers.
