Job Description:

Provide high-quality, patient-centric, cost-effective inpatient-level care in the home setting. Please note this is not a remote position. This is a virtual position based out of our Command Center in Charlestown, MA.

1. Achieve clinical outcomes equal to or better than inpatient hospitalization.
2. Work in a team environment with physicians, NPs, and RNs in a virtual Command Center.
3. Medically Home significantly reduces total medicine expenses from avoidable inpatient visits, post-acute care, and readmissions.
4. Enhance patient experience.
5. Utilize state-of-the-art equipment at the Command Center and patients’ homes.
6. Provide services such as home infusion, meals/nutrient consultations, pharmacy/medical management, mobile imaging, laboratory, and DME/oxygen.
7. Coordinate integrated care with primary care providers, specialists, and ancillary departments.

Minimum Qualifications:

Licensure, Certification, Registration:

• BC/BE in Internal Medicine, Family Medicine, or Geriatrics.

Skills, Knowledge & Abilities:

• Experience as a hospitalist or SNF is a plus.
• Commitment to providing high-quality, evidence-based medicine to a diverse patient population.
• Excellent clinical and communication skills.
• Ability to work effectively in a team environment.
