II. Position Summary:

The Virtual Home Hospitalist is a core member of the UMass Memorial Medical Center (UMMMC) Hospital at Home (UMMMC HAH) clinical team and plays a central role in admitting, transferring and caring for patients from the UMMMC University, Memorial, Pavilion campus emergency departments and med/surge floors to the UMMMC HAH program. After an initial in-person training and onboarding process in the Worcester based command center (located at 306 Belmont Street), the Virtual Home Hospitalist may practice virtually provided they are providing care within the state of Massachusetts, have a HIPPA secure private space to provide care, have stable high-speed internet connection and are available to come into the command center in Worcester for medical student, resident, attending trainings and should urgent issues arise where this is required clinically for any reason. It is also expected that the Virtual Home Hospitalist will at a minimum occasionally practice in the Worcester based command center to build relationships with UMMMC HAH caregivers and participate in shadowing and training of new recruits (i.e. practicing clinically in-person from the command center several days per month). In their role, the Virtual Home Hospitalist will closely collaborate with the UMMMC HAH nursing and paramedic caregivers to provide telehealth facilitated inpatient level care in the home for patients requiring ongoing inpatient level acute care. On a routine basis, the Virtual Home Hospitalist (a) provides clinical consultation and support to the HAH RN screening patients for admission to the UMMMC HAH program (b) facilitates admissions to the UMMMC HAH program (c) serves as the primary responding clinician and caregiver for patients admitted to the UMMMC HAH service and (d) models the highest standards of care for an often social and medically vulnerable population.


III. Major Clinical Responsibilities:

1. Provides direct patient care during 12-hour daytime shift (7:00a-7:00p)

2. Provides emergency overnight coverage support to HAH APP/RN/MIH caregivers (7:00p-7:00a) on 50% of on active daytime call days which may include after-hours phone calls, telehealth video visits, orders and after-hours patient escalations.

3. A 1.0 full time equivalent will initially be defined as 160 (7:00a-7:00p) shifts per year inclusive of emergency overnight coverage on 50% of nights with an overnight coverage stipend with goal to move towards elimination of overnight coverage requirement and overnight coverage stipend via development of dedicated night coverage team.

4. Supports all UMMMC HAH new patient acquisition efforts led by the UMMMC patient acquisition RN team during hours of new patient referral acceptance with clinical consultations and close communication with referring UMMMC caregivers.

5. Performs “admission tuck-in” video visits with patients on day of admission with RN or paramedic in the home and prioritizes medication reconciliations, patient and family collaborative care planning and proactive safety planning.

6. A typical daily schedule include:

7:00 am Shift starts

7:00-7:30 am Multidisciplinary rounds

7:30-8:30 am Admission consultations

8:30-1:00 pm Patient video visits, orders, specialty consults, documenting

2:00-7:00 pm Admission consultations, “admission tuck-in” video visits

7:00 pm – 7:00 am Overnight coverage for urgent needs (50% of nights)

7. Serves as a key contributor in identify opportunities to improve program care processes

8. Will provide supervisory services to residents and APPs working within the HAH program

9. Highly collaborative caregiver closely partnering with nursing, paramedic, APP teams with excellent verbal and written communication skills

10. Joins and contributes to regularly scheduled UMMMC HAH and hospitalist team meetings

11. After initial clinical training and onboarding, the UMMMC Virtual Home Hospitalist will have the option to provide clinical care working remotely from home office provided this care is provided in HIPPA compliant space with privacy for video visits and required availability to nursing, paramedic, APP care team. The UMMMC Virtual Home Hospitalist will be asked to work from 306 Belmont command center in Worcester from time to time to participate in clinical conferences, trainee education, mentoring and clinical care.

12. Equitably participates in clinical coverage over weekends (two weekends per month) and holidays (two major holidays and one minor holiday per year).

13. The UMMH Virtual Home Hospitalist will have the opportunity to work clinically both as a Virtual Home Hospitalist and Brick/Mortar Hospitalist if interests and service needs align for the candidate, the UMMMC HAH program and hospital medicine programs

IV. Position Qualifications:

License/Certification/Education:

Required:

1. MD or DO degree

2. MD or DO licensure in the State of Massachusetts

3. BC/BE Internal Medicine or Family Medicine

4. BLS and ACLS certification


Experience/Skill:

Required:

1. Recent inpatient level acute care experience

2. Comfort with telehealth or growth mindset to learn telehealth

3. Comfort with closely partnering with patient, family, unskilled and skilled caregivers in the home to create and execute care plans

4. Comfort with care of the complex geriatric patient in low control environments

5. Comfort with uncertainty and holistically balancing risks of brick/mortar hospitalization and home hospitalization to provide second-to-none-quality care

6. Highly collaborative, passion for working with small, dedicate care team

7. Ability and interest to pro-actively manage a panel of inpatient-level acutely ill patients using combination of remote- and in-home resources
