Job summary

Qualifications

• 3+ years RN experience in complex care, care management, or transitions of care.
• Experience with Medicare Advantage, managed care, or home-based care.
• Active CA RN license and BLS certification.

Responsibilities

• Coordinate care for medically complex Medicare Advantage members.
• Lead transitions of care post-discharge from hospital or SNF.
• Perform medication reconciliations and monitor clinical changes.
• Collaborate with multidisciplinary teams for coordinated care.
• Support chronic disease management and care gap closure.
