Job Description

The Vice Chair and Enterprise Lead for Community Medicine will provide academic and health system leadership to meet the needs of communities affected by poverty, racism, and social injustice. The candidate will drive regional health equity and population health by building systems that address social needs and health equity for the regional population served by Jefferson, which provides comprehensive, longitudinal care through primary care attribution and empanelment. This population includes 560,000 primary care patients in the greater Philadelphia region, over 100,000 on Medicaid, and more than 20 community-based sites in low-income neighborhoods.

The candidate will report to the Chair of Family and Community Medicine/Enterprise Chief of Primary Care and the Executive Vice President for Health Equity and Community Engagement.

Requirements: Physician with 5-10 years of clinical care experience in community-based settings serving low-income populations, with demonstrated success in developing and implementing clinical models and initiatives, and working effectively with community coalitions and partners.

The successful candidate will collaborate with EVP Health Equity, Chair/Enterprise Chief of Primary Care, and other key leaders to:

  1. Develop and execute strategies for addressing social needs in clinical care, emphasizing excellence in care delivery for attributed and primary care populations.
  2. Ensure high-quality implementation of social needs approaches across Jefferson’s attributed/empaneled populations.
  3. Identify disparities in health processes and outcomes, and partner with clinical and population health leaders to address these focus areas.
  4. Create complex care models for individuals with high social needs, focusing on primary care as the foundation of continuous, comprehensive care, and work with clinical, operational, and financial leaders to sustain these models.
  5. Grow grant portfolios aligned with the needs of Jefferson’s Community Medicine sites, including collaborative grants with community organizations.
  6. Collaborate with Jefferson Health Plan and other insurers to develop community-based care models.
  7. Ensure community-centeredness in new clinical care initiatives.
  8. Promote an inclusive, transparent, and supportive work culture for patient-facing teams.
  9. Advocate for clinical teams working in Jefferson’s community-based primary care sites.
  10. Support the academic and professional development of colleagues through mentorship and organizational engagement.
  11. Encourage opportunities in Community Medicine for students, residents, and learners.
  12. Identify research and evaluation opportunities within Jefferson Health and with community organizations.
