Job summary

U.S. Department of Veterans Affairs is seeking a Social Worker (GS-11) to provide intensive case management and psychosocial services to Veterans and families within the Wilkes-Barre VAMC catchment area.

The role requires an MSW from a CSWE-accredited program, state licensure, and full English proficiency. Eligible for EDRP; travel may be required; typical work involves collaboration with inpatient and outpatient teams to optimize patient outcomes.

Qualifications

• Master's degree in social work from a CSWE-accredited program is required.
• Licensure or certification to independently practice social work at the master's level is required.
• Proficiency in spoken and written English is required.

Responsibilities

• Provide intensive psychosocial assessments and case management to Veterans and families.
• Coordinate care with inpatient and outpatient teams to ensure continuity of care.
• Document clinical work in MHICM and Medical Center records per policy.
• Assist in referrals to community resources and other governmental agencies.
• Travel within the Wilkes-Barre VAMC catchment area to conduct interviews and provide services.
