Job summary

Qualifications

• Proven ability to assess psychosocial needs and coordinate care for Veterans.

Responsibilities

• Lead CCICM and PACT duties with high-level psychosocial assessment and care planning.
• Provide short-term counseling, crisis intervention, and care coordination.
• Develop policies and supervise multidisciplinary staff within the program.
• Monitor and report program performance, budgets, and outcomes.
