Job summary

U.S. Department of Veterans Affairs is seeking a Social Worker Program Coordinator to develop and coordinate VDC programs in the WCHCS catchment area. The role requires licensure at the master’s level, and leadership for multidisciplinary teams in a community-based care setting.

The position emphasizes adherence to VHA guidelines, supervision, and the ability to deliver training and policy development while coordinating with VISN and VA Central Office as needed.

Qualifications

• Must be licensed or certified to independently practice social work at the master's degree level.
• Ability to supervise multidisciplinary staff and provide training.
• Knowledge of program coordination, administration, and monitoring.

Responsibilities

• Establishes and maintains effective working relationships with Veterans, staff, and partners.
• Oversee consultation and facilitation of agreements with Office of Community Care for VDC.
• Develop, coordinate, and evaluate VDC in the WCHCS catchment area.
• Ensure compliance with VHA guidelines and program policies.
