Job summary

Heritage Behavioral Health Center in Decatur, Illinois, is hiring a Community Support Specialist – Veteran Focused to deliver person-centered services that improve independence in the community. You will coordinate care with a multidisciplinary team to support veterans and non veterans alike.

Responsibilities include individual services, skill-building (medication management, budgeting, housing, meal planning, transportation), and helping clients access housing, employment, benefits, and VA

Qualifications

• High School Diploma or GED required; Bachelor's degree preferred.
• Experience working with individuals with behavioral health challenges preferred.
• CRSS certification or willingness to obtain, if applicable.

Responsibilities

• Provide individualized, person-centered services designed to improve independent functioning in the community.
• Deliver skill-building interventions in areas including: medication management, budgeting, household management, meal planning, transportation, healthcare coordination, and social/communication skills.
• Monitor mental health and substance use symptoms and support stabilization efforts.
• Assist individuals in accessing housing, employment, benefits, and community resources.
• Coordinate care alongside a multidisciplinary treatment team.
• Support Veterans in navigating VA services and community resources.
