Job Summary

The Veterans Case Manager supports housing and stability for Veteran households in Salt Lake County through the SSVF program. Responsibilities include identifying housing-insecure individuals, assessing eligibility, and assisting with housing stabilization. The role involves meeting clients in the community and their homes, requiring organizational skills and the ability to manage multiple responsibilities efficiently.

Grade 6, $18.90/hour

Shift
Monday – Friday, 40 hours

Responsibilities
  • Create tailored housing plans considering individual barriers and goals.
  • Manage caseloads independently, tracking eligibility and prioritizing tasks.
  • Maintain professional relationships with community partners and emergency services.
  • Provide direct case management, including crisis intervention and resource connection.
  • Participate in meetings and maintain accurate records and reports.
  • Promote community education and participate in outreach activities.
  • Operate and maintain agency vehicle and supplies.
  • Contribute to understanding and respecting diverse populations, using Trauma-Informed Care principles.
  • Perform other duties as assigned.

  • Demonstrate awareness of diverse populations.
  • Support agency commitment to awareness and appreciation of diversity.
  • Embrace housing first, harm reduction, and low barrier access as best practices.
  • Maintain professional boundaries with clients, guided by Trauma-Informed Care.
Qualifications
  1. Proficiency in Microsoft Office Suite.
  2. Ability to collaborate with community partners.
  3. Successful completion of background check and drug screening.

Education & Experience

  • Lived, work, or volunteer experience required, or a relevant degree.
  • Experience with vulnerable populations required.
  • Experience with mental health/substance use disorders preferred.
  • Knowledge of homeless populations and resources preferred.
Physical & Equipment Requirements
  • Assist clients with moving, including lifting furniture.
  • Perform outreach in various weather conditions, including walking up to five miles daily.
  • Stand and walk for extended periods.
  • Use stairs, reach, bend, and perform physical activities as needed.
  • Hold a valid driver's license and be at least 21 years old to operate agency vehicles.
