Job summary

STGi is seeking a Registered Nurse Care Manager to provide services at our Community Based Outpatient Clinic in Uniontown, PA. The RN coordinates care with VA facilities and other stakeholders, focusing on service excellence and patient advocacy.

The role requires clinical expertise, PA license, and a minimum of three years' experience in medical-surgical/outpatient settings, with CPR/BLS certification and strong communication skills.

Qualifications

• RN with active Pennsylvania license.
• Minimum three years in medical-surgical/outpatient/primary care.
• CPR/BLS certification required.

Responsibilities

• Coordinate patient care across clinic and VA facilities.
• Provide nursing care per orders and supervise staff when applicable.
• Lead team huddles and ensure smooth patient flow per access standards.
• Perform triage for walk-in patients and manage phone inquiries.
• Administer medications and assist providers as needed.
• Document patient information in EMR and maintain confidentiality.
