Job summary

The Licensed Practical Nurse (LPN) position at Syracuse VA Medical Center's Community Living Center delivers patient care under VA practice and licensure, responding to emergencies, administering medications, and documenting actions to support safety and dignity.

As part of the nursing team, you collaborate with RNs and MDs, provide education to patients and families, and help ensure infection control and privacy while adapting care to changing conditions in a long-term care setting.

Qualifications

• Graduate of an approved practical or vocational nursing program.
• Active, full, current, unrestricted LPN licensure in the United States or DC.
• Graduation from an NLNAC-accredited program may be required.
• Ability to document care and support infection prevention practices.
• English language proficiency for direct patient care.

Responsibilities

• Manages patient/resident care issues per VA practice and LPN licensure.
• Maintains knowledge to care for patients with acute and chronic conditions.
• Provides full range of practical nursing care for various physical and behavioral problems.
• Recognizes and intervenes in emergencies (Rapid Response, Code Blue).
• Prepares, administers, and documents medications and therapies.
• Promotes privacy, dignity, and respectful interactions with patients and families.
