The RN Care Coordinator functions as part of a multidisciplinary team, including physicians, social workers, discharge planning assistants, and payers. This position ensures that patient's progress during the acute episode of care through post-discharge is quality-driven, efficient, cost-effective, and safe. It is patient-focused and outcome-oriented.
Licensure, Certification, or Registration Requirements for Hire:
- Licensed Registered Nurse in the State of Virginia or eligible
Licensure, Certification, or Registration Requirements for continued employment:
- Current RN licensure in Virginia
- Complete 15 Continuing Education Units per year
Experience REQUIRED:
- Minimum of three (3) years of nursing experience in an acute care setting
Experience PREFERRED:
- Academic healthcare experience
- One (1) year of UM/Care Coordination experience
- Clinical experience with specialty patient populations
Education/training REQUIRED:
- Baccalaureate Degree in Nursing from an accredited School of Nursing, or a Bachelor's degree in a biological, health, or social sciences field from an accredited school
Education/training PREFERRED:
- Master's Degree in Nursing or a healthcare-related field from an accredited program
Independent actions required:
- Works with attending and consulting physicians to facilitate effective and efficient hospitalization transitions
- Collaborates with the multidisciplinary team to meet patient needs, coordinate care, ensure appropriate reimbursement, and utilize resources effectively
- Seeks social workers' expertise for psychosocial issues and complex discharge planning, interacting with patients, families, healthcare professionals, and community agencies
- Acts as a liaison between the hospital and community agencies for clinical and referral information exchange
- Maintains hospital compliance with QIO and CMS guidelines
- Uses medical necessity criteria to assess care appropriateness, assists in denial and appeals, and reports risk management issues
- Participates in and completes all patient safety initiatives relevant to the role
- Performs all responsibilities in alignment with VCUHS’ Mission and Values
- Establishes and maintains professional relationships with providers, patients, families, and internal staff
