Job summary

Diversicare is seeking a dedicated Certified Nurse Assistant to join our post-acute care team at our Oak Ridge facility. You will support residents with daily living activities and accompany them to appointments.

The role requires safe resident transfer, a valid state CNA certification, clean driving record, and reliable transportation. We offer comprehensive benefits and a culture of service excellence.

Qualifications

• Must be physically able to assist ambulatory and non-ambulatory residents transfer to and from wheelchair.
• Valid state driver’s license required.
• Certified Nurse Assistant (C.N.A.) qualification.
• Clean driving record.
• Clean criminal record.
• Any other state required certifications.

Responsibilities

• Ensure resident safety during transport to appointments.
• Accompany residents to medical visits; stay with them until completed.
• Sign in residents and present documents at visits.
• Obtain new prescriptions and documents before leaving doctor’s office.
• Report back to facility after visits and be prepared to answer questions.

Responsibilities

• Assure the safety of each resident when transporting to and from the doctor’s office and/or hospital.
• Accompany each resident on all medical visits. Stay with resident(s) until visit is complete.
• Observe the following guidelines at each visit:
• Sign in resident upon entrance.
• Present resident’s documents to office personnel.
• Before leaving doctor’s office, please observe the following:
• Obtain resident’s documents.
• Obtain any new prescriptions.
• Check to make sure doctor has written on the doctor’s order form.
• If follow-up appointment necessary, obtain appointment cards.
• After returning to facility, report to RCM and be prepared to answer questions concerning visit.
• Performs other duties as required and assigned.

Qualifications

• Possess a valid state driver’s license.
• Qualified as a Certified Nurse Assistant (C.N.A.).
