Responsibilities
  1. Assure the safety of each resident when transporting to and from the doctor’s office and/or hospital.
  2. Accompany each resident on all medical visits. Stay with resident(s) until visit is complete.
  3. Observe the following guidelines at each visit:
    • Sign in resident upon entrance.
    • Present resident’s documents to office personnel.
  4. Before leaving doctor’s office, please observe the following:
    • Obtain resident’s documents.
    • Obtain any new prescriptions.
    • Check to make sure doctor has written on the doctor’s order form.
    • If follow-up appointment necessary, obtain appointment cards.
  5. After returning to facility, report to RCM and be prepared to answer questions concerning visit.
  6. Performs other duties as required and assigned.
Qualifications
  • Physically able to help ambulatory and non-ambulatory residents transfer to and from wheelchair to vehicle.
  • Possess a valid state driver’s license.
  • Qualified as a Certified Nurse Assistant (C.N.A.).
  • Clean driving record.
  • Clean criminal record.
  • Any other state required certifications
