Job summary

Cone Health is seeking a VBCI Population Health RN Care Manager to drive value-based care through comprehensive case management and care coordination. You will collaborate with interdisciplinary teams to create individualized care plans for diverse populations and ensure smooth transitions across care settings.

You will educate patients, engage families, and apply motivational interviewing and teach-back techniques to improve health outcomes.

Qualifications

• Graduate from Specialty Training Program - Nursing.
• Active RN license in the practicing state is required.
• BSN is preferred; CCM certification is a plus.

Responsibilities

• Case management and care coordination with interdisciplinary teams.
• Liaise between patients, families, and providers to ensure continuity of care.
• Educate patients and families on conditions, options, and self-management.
• Employ motivational interviewing and teach-back methods.
• Assess and monitor health status; identify barriers to care.
• Review and adjust care plans based on progress and data.
• Support quality measures, care gap closure, and HEDIS metrics.
• Navigate community resources and coordinate care transitions.
• Document care activities in the EHR and report outcomes.
