Qualifications

• Board-certified or board-eligible in Family Medicine or Internal Medicine.
• Active Texas medical license or ability to obtain prior to start.
• Active DEA registration or ability to obtain.

Responsibilities

• Provide care for a Medicare Advantage population within a value-based care model.
• See 14–16 patients per day with emphasis on comprehensive, patient-centered care.
• Collaborate with a multidisciplinary team to address physical, behavioral, and social health needs.
