Job summary

CareMore Health in Green Valley, AZ seeks a Primary Care Physician to manage complex patients within a value-based care model. You will work with an interdisciplinary team to focus on prevention and durable patient relationships, with relocation support and a 4-day work week option.

Physicians will join a team including NPs, Care Managers, Pharmacists, Social Workers and Support staff, sharing the load to place patient care first. Board eligibility and AZ license are required.

Qualifications

  • MD/DO from an accredited medical school.
  • Active Arizona medical license.
  • DEA registration.
  • Board certified or eligible.
  • Interest in value-based, patient-centered care.

Responsibilities

  • Manage patients with complex health needs alongside a multidisciplinary team.
  • Collaborate with Nurse Practitioners, Care Managers, Pharmacists, Social Workers and Clinical Support Teams.
  • Provide patient-centered care focusing on prevention and lasting relationships.

Education
