Value-Based Practice Improvement Specialist
Overview
The Value-Based Practice Improvement Specialist represents and advances RWJBarnabas Health's interest as the preferred and ultimate preeminent health system through superior customer service and execution of Physicians' value-based and quality strategies.
The Value-Based Practice Improvement Specialist represents commitment to developing and managing high performance network and to engaging various Physicians' value-based and quality improvement strategies including but not limited to Quality Improvement, Cost Optimization, Clinical Efficiency, Administrative Efficiency, and Population Health.
Responsibilities
- Serves as health system liaison working with Physicians/Physician groups and payers in a specified geographical location. Will be the main point of contact for the respective physician/physician groups for all issues including quality performance, referral pattern, patient outreach, cost optimization, and building relationship.
- Participate in various Organization Projects to build a better Health System.
- Conducts Physician onboarding in various value-based programs; the onboarding process includes but not limited to a comprehensive orientation of the health system, array of benefits, physician's responsibilities, etc.
- Responsible for monitoring and coordinates recurring meetings with Physician/Physician groups to go over their quality performance, gaps in care, chronic fall-out outreach, and financial performance; to create strategic improvement plans.
- Contribute creative solutions and ownership of daily assignments for seamless communication and systematic completion of routine and special projects. Follow protocols and use common sense to escalate situations to management appropriately.
- Participates in meetings by providing analysis, feedback and recommendations that foster a successful relationship between RWJBarnabas Health System, essential Physicians, and external agencies.
- Works cohesively with other departments with both clinical and non-clinical personnel to provide the best support for the Physicians to perform well.
Qualifications
- Associate Degree or equivalent work experience
- Two to four years of healthcare experience
- Must possess strong interpersonal, communication, management, and technology skills
- Must demonstrate team building and collaborative practice behaviors
- Must demonstrate understanding of performance improvement and quality outcomes management
- Physician office operations experience
- Ability to travel various part of New Jersey to visit the Physician/Physician Groups
- Demonstrate ability to multi-task, prioritize, and use good judgement in a fast-paced environment
- Strong presentation and problem-solving skills; familiar with root cause analysis
Essential Job Functions

Scheduling Requirements
- This role requires regular travel to various parts of New Jersey (Southern & Central Regions) to visit the Physician/Physician Groups.
