Job summary

Partners in Digital Health in Seattle, WA is seeking an experienced Nurse Practitioner to join a well-established value-based primary care clinic. The NP will manage a diverse patient panel including pediatric, adult, and geriatric patients, with a mix of well visits and acute care.

Qualifications

• Certified Family Nurse Practitioner
• In the past 5 years, NP with >=2 years in outpatient primary care
• OR 1 year in outpatient primary care plus 1 year in urgent care or fellowship

Responsibilities

• Manage your own patient panel with acute, chronic, and preventive care
• Treat patients in the office, testing centers, and via telehealth
• Participate in weekly Clinical Rounds and ongoing learning opportunities
• Collaborate daily with in-office teammates during huddles and care discussions
• Perform in-office procedures as appropriate
