Valeo Occupational Therapist Assistant/COTA - PT/PRN

Responsibilities include evaluating, planning, and administering occupational therapy modalities/regimens to patients in their homes to develop and restore function through rehabilitative occupational therapy as prescribed by a physician. The role also involves supervising occupational therapist assistants.

  1. Perform accurate and comprehensive assessments.
  2. Complete OASIS data sets accurately.
  3. Obtain physician orders for certification and recertification of home occupational therapy services.
  4. Communicate changes in patients’ clinical status to healthcare team members and HHA office staff promptly, both verbally and in writing.
  5. Ensure assessment results, recommendations, and physician orders are communicated thoroughly and accurately.
  6. Coordinate care planning and delivery with physicians, healthcare team members, and HHA staff to optimize patient outcomes.
  7. Deliver occupational therapy modalities appropriate to the patient’s clinical status and needs, adhering to standards of care, laws, regulations, and standards.
  8. Assess the effectiveness of occupational therapy services and implement processes to support optimal outcomes.
  9. Develop tailored home exercise programs based on the patient’s needs and age.
  10. Assess and reassess patient pain appropriately.
  11. Utilize suitable pain management modalities, including occupational therapy modalities.
  12. Provide therapy in a safe environment that minimizes errors.
  13. Support a culture of safety and quality.
  14. Educate patients and families on pain management techniques.
  15. Complete patient documentation timely.
  16. Attend in-service and staff meetings as required.
  17. Complete assigned training and annual education requirements on schedule.
  18. Maintain patient confidentiality at all times.
  19. Perform other duties as assigned.

Qualifications:

  • Current valid state occupational therapy license.
  • Graduate of an accredited occupational therapy program; or eligible for the National Registration Examination.
  • At least two years of work experience, with preferred home health experience.
  • Valid driver’s license, reliable vehicle, and proof of insurance.
  • Current TB test, criminal background check, and CPR certification.
