Job Description

The Hospice CNA performs essential personal care duties for hospice patients in their homes, including:

1. Performing duties as assigned by supervising RN, such as baths, hair care, oral hygiene, nail care, foot care, skin care, toileting, pressure relief, dressing, feeding, and transferring patients.
2. Taking accurate vital signs and recording them.
3. Assisting patients with activities of daily living and demonstrating understanding of hospice philosophy.
4. Notifying licensed staff of patient condition changes or pain complaints.
5. Using techniques to assist with mobility and positioning.
6. Supporting adequate nutrition and fluid intake.
7. Ensuring patient safety and reducing errors.
8. Respecting patient privacy and needs, supporting a safety and quality culture, and following infection control procedures.
9. Recognizing emergencies and communicating findings.
10. Charting according to standards and working as part of the interdisciplinary team.
11. Completing documentation, attending meetings, and fulfilling training requirements.
12. Maintaining confidentiality and performing other duties as assigned.

Minimum Qualifications

• High School diploma or equivalent (preferred).
• Hospice Aide certificate, Nurse aide training, and current registry listing.
• Valid driver’s license, reliable vehicle, insurance, and willingness to operate personal vehicle.
• Current TB test and CPR card.
• No findings of abuse or neglect on the nurse aide registry.
