Job summary

The U.S. Department of Veterans Affairs seeks a VA/DoD Liaison to facilitate seamless health care transitions for injured and ill Service members and Veterans from DoD MTFs to VA facilities.

On-site collaboration with MTF teams, clinicians, and enrollment staff is required to ensure timely access to VA benefits and services. Responsibilities include coordinating discharge planning, educating patients about VA resources, registering individuals in VA systems, and reporting on transition mechanics

Qualifications

• Education verification required for foreign or domestic qualifications.
• Knowledge of VA health care benefits, DoD/VA policies, and federal regulations.

Responsibilities

• Works closely with MTF treatment team to coordinate discharge planning to VA facilities.
• Educates service members and families about VA health benefits and resources.
• Registers active duty personnel or enrolls veterans in the VA system using referral information.
• Collaborates with MTF social workers, nurses, case managers, and discharge planners to identify transfer-ready patients.
• Prepares reports and presentations on the seamless transition process for VA staff and DoD personnel.
