Job summary

Northwestern Medicine is seeking a part-time Case Manager, RN to support Utilization Review at Catherine Gratz Griffin Lake Forest Hospital. The role offers flexible weekday and weekend coverage, with shifts 6a–6p or 6p–6a.

The RN will contribute to discharge planning, care coordination, and compliance with regulatory standards in a fast-paced hospital setting. Ideal candidates hold an Illinois RN license with a BSN and several years of clinical experience, and may pursue an MS in Nursing.

Qualifications

• Illinois RN license and BSN required.
• Minimum 3 years of related clinical experience.
• Ability to coordinate care and support discharge planning.

Responsibilities

• Develops individualized treatment plans with patients and families.
• Leads care conferences for patients with stays over 7 days.
• Documents and refers issues to risk management, infection control and quality.
• Assists discharge planning and coordinates transfer to appropriate care levels.
• Maintains CMS and regulatory compliance through chart audits and documentation.
