Northwestern Medicine Catherine Gratz Griffin Lake Forest Hospital is seeking a Registered Nurse to support Utilization Review in a casual, on-call capacity. The position offers flexible weekday/weekend coverage with 6a–6p or 6p–6a shifts, and a pathway to develop in case management and discharge planning within a leading medical system.

Qualified candidates hold an Illinois RN license, a BSN, and at least 3 years of clinical experience.

Formación

• Registered Nursing license issued by the State of Illinois.
• Bachelors of Science Nursing.
• 3 or more years of related clinical experience.

Responsabilidades

• Develops individualized treatment plans for patients in collaboration with the patient and family.
• Coordinates and leads care conferences on patients with length of stay greater than 7 days.
• Documents unusual occurrences and patient relations issues; makes referrals to risk management, infection control and quality departments.
• Assesses patient and family needs for discharge services; coordinates transfer to appropriate levels of care.
• Documents discharge planning on the interdisciplinary plan of care and education record.

Bachelors of Science Nursing

Masters of Science Nursing

Certification in Case Management (ACM)

Membership in Professional Nursing Organization
