Job summary

AbsoluteCARE Medical Center & Pharmacy seeks a Supervisor to provide day-to-day operational support for Transitional Care and Complex Care Management teams, coordinating work schedules, ensuring policy compliance, and monitoring the quality of care delivered to members.

The role partners with the Director/Manager, Shared Services, Medical Center, and other stakeholders to facilitate seamless care transitions across inpatient, ambulatory, and community settings, while coaching staff and driving

Qualifications

• Associate degree required; RN license active and unrestricted.
• Case management certification or equivalent strongly preferred.
• BLS/CPR certification required.

Responsibilities

• Supervise Transitional Care Managers and Care Navigators teams.
• Ensure adherence to care management standards and documentation protocols.
• Lead staff training, 1:1 meetings, and performance reviews.
• Review program KPIs and implement corrective actions.
• Coordinate interdisciplinary care team meetings to support member outcomes.
• Maintain HIPAA compliance and protect member privacy.
