Our selected candidate will coordinate and deliver care that includes:• Patient assessments and evaluating the effectiveness of care• Diagnostic studies, admits, appointments, procedures, and consultant/referrals• Serving various populations from infants and pediatric to adult and geriatric• Interacting directly with physicians, patients, staff Facilitating home health and other outpatient therapies• Ensuring compliance and/or adjustments to treatment plans• Patient advocacy and education• Delegating appropriate responsibilities to staff as needed• Ensuring all performance, care, safety compliance• And more

Requirements:
