Position Description

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies, and all relevant regulations.

Qualifications

• Long Term Care experience
• Point click care charting experience
• Current LVN/LPN license in practicing state
• Current BLS/CPR certification or willingness to acquire one
• Experience in nursing management is preferred but not required

Responsibilities

• Follow established procedures for charting and incident reporting
• Adhere to company procedures for narcotic counting, pharmacy orders, and restraint reduction
• Document any changes in patient’s condition, including nutritional issues, infections, skin conditions, weight loss, or other noticeable changes
• Utilize the Quality Life Manual for patient identification, placement, and monitoring
• Notify relevant departments of room changes, LOA, new admits, or discharges
• Make rounds with physicians as necessary
