Job summary

Qualifications

• Current RN License in state of practice.
• BLS for healthcare providers.
• Unit specific certification as required (ACLS, PALS).
• Ability to communicate effectively both verbally and in writing.
• RNs with less than 12 months of working experience as an RN prior to joining Intermountain must obtain their BSN within five years of their start date.
• BSN preferred from an accredited institution.

Responsibilities

• Assess physical, social, and psychological status; include cognitive and communication abilities as appropriate.
• Collaborate with interdisciplinary team to develop individualized plan of care.
• Involve patient, family, and team to educate and plan necessary care transitions.
• Direct or delegate care to the care team in alignment with the plan of care.
• Set priorities, coordinate resources, and manage time to deliver care.
• Evaluate patient response to interventions and adjust the plan as needed; maintain professional documentation.
