BAYADA Home Health Care is seeking a Registered Nurse (RN) to join our home health visit team in Vermont. You will provide skilled nursing and rehabilitative care to clients at home, collaborating with a multidisciplinary team to ensure compassionate, high-quality service.

Ideal candidates are experienced in acute and post-acute care, can work independently, and manage a dynamic visit schedule with strong documentation skills. BAYADA offers weekly pay and comprehensive benefits.

Qualifications

• Current nursing license in good standing in Vermont.
• Graduation from an accredited nursing program.
• At least 1 year of adult/geriatric healthcare experience in acute, emergency, home health, hospice, ICU, med-surg, sub-acute or rehab within last 2 years.

Responsibilities

• Perform head-to-toe assessments and identify subtle changes in patient condition.
• Deliver skilled nursing and rehabilitative care in the home setting.
• Triage and independently manage urgent clinical situations at the home.
• Conduct medication reconciliation, patient teaching, and safety monitoring.
• Perform comprehensive wound assessment, care, and documentation.

BAYADA Home Health Care is currently seeking a Registered Nurse (RN) to join our home health visit team. You will be an integral member of a multi-disciplinary health care team that provides skilled nursing and rehabilitative care to clients, affording them the opportunity to remain at home. How You’ll Make an Impact:

• Use strong head-to-toe assessment skills with the ability to identify subtle changes in patient condition
• Utilize your experience with acute and post-acute care (e.g., CHF, COPD, wound care, IV therapy, post-surgical care)
• Triage and manage urgent clinical situations independently in the home setting
• Perform medication reconciliation, teaching, and safety monitoring
• Perform comprehensive wound assessment, care, and documentation

Qualifications for a Registered BAYADA Nurse:

• A current nursing license in good standing in the state of Vermont
• Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma
• At least one year of current or recent hands-on medical experience within the past two years with the adult/geriatric population in acute care, emergency department, home health, hospice, ICU, med surg, sub-acute care or rehab units (medical or skilled nursing settings).
