Job description

The Center for Human Development, (CHD) is seekingan Outreach Where you Live ( OWLS) Unsheltered Outreach Caseworker to join our new Unsheltered Outreach program in Litchfield county area.

Your role as an OWLS Unsheltered Outreach Caseworker:

The OWLS Unsheltered Outreach Caseworker oversees a variety of cases through different venues on a daily basis in the Litchfield county area and is able to provide the unsheltered seeking assistance the necessary tools to accomplish their goals. They will maintain a schedule including but not limited to hours of actual street outreach, use of equipment and supplies as well as a schedule of external support providers that may able to further the individuals’ attempts to end their homelessness. Also will work within the program’s model as well as protocols established by the Western CT Mental Health Network and the Coordinated Access Network with whom we jointly run the outreach services establishing open communication, support and accountability among collaborating agencies.

REQUIRMENTS:

  • High school diploma or GED required
  • Two years in a human service organization required
  • Two years’ experience working with adults with severe and prolonged mental illness, addiction/recovery or unsheltered populations
  • Individuals with lived experience encouraged to apply
  • Bachelor’s degree from an accredited educational institution preferred
  • Comfortable being in the field and covering Litchfield county, this is not an office position and requires a large amount of driving
  • Ability to utilize technology to perform essential job duties
  • Reliable, registered & insured vehicle and approved driving record check
  • Approved criminal background check (CORI)

Shifts available:

  • Full time: Litchfield, CT
  • Schedule: 2:00pm-10:00pm on Tuesday, Wednesday, Friday, and Saturday. 11:00am-7:00pm on Thursday. Sunday and Monday off.
