Convalarium Care Center is seeking a Unit Manager - LPN to lead our nursing team in delivering high-quality resident care and to oversee day-to-day clinical operations.

The ideal candidate possesses a valid LPN license, strong leadership, and a commitment to regulatory compliance, quality improvement, and staff mentorship. This on-site role offers growth opportunities within a collaborative healthcare setting.

Qualifications

• Must possess a current, unencumbered, active license to practice as a Licensed Practical Nurse in this state.
• Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served.
• Must be able to read, write, speak, and understand the English language.
• Must be a supportive team member, contribute to and be an example of team work and team concept.
• Must possess the ability to make independent decisions when circumstances warrant such action.
• Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to nursing care facilities.
• Must possess leadership and supervisory ability and the willingness to work harmoniously with and supervise other personnel.
• Must be willing to seek out new methods and principles and be willing to incorporate them into existing nursing practices.

Responsabilités

• Provide direct nursing care to residents, as well as supervise and direct the day-to-day nursing activities performed by STNAs.
• Ensure that all assigned nursing personnel, including STNAs and other nursing staff, comply with current federal, state, and local standards, guidelines, and regulations.
• Develop and implement individualized care plans for residents, in collaboration with the interdisciplinary team.
• Coordinate and monitor resident care, including admission, transfer, and discharge processes.
• Ensure that all nursing service personnel follow established policies and procedures, and report any deviations to the Nurse Supervisor and/or Unit Manager.
• Participate in quality assurance programs, including surveys and inspections.
• Collaborate with other departments, including dietary, social services, and activities, to ensure a comprehensive approach to resident care.
• Provide education and training to nursing personnel, as needed.
• Conduct performance evaluations, and provide feedback and coaching to improve staff performance.
• Maintain accurate and detailed records of resident care, including charts, reports, and other documentation.
