Job summary

Hackensack Meridian Health is seeking a Unit Based Pharmacist to join our patient-centered team. This role integrates pharmacists into critical care with responsibility for clinical and distributive duties within the pharmacy department.

The candidate will validate medication reconciliation on admission and before discharge, educate patients, and work with physicians to optimize therapy while managing costs. A NJ license and PGY1 completed are required; ACLS within 6 months is expected.

Qualifications

• Bachelor's Degree in Pharmacy from an accredited college.
• Strong interpersonal and teamwork abilities; excellent written and verbal communication.
• Proficient with Microsoft Office and Google Suite.

Responsibilities

• Validate medication reconciliation: reconcile medications on admission and before discharge to ensure continuity of care.
• Perform patient assessment and counseling; educate on new medications and high-risk regimens.
• Facilitate discharges; address barriers to medication adherence with the interdisciplinary team.
• Collaborate with doctors to optimize cost-effective, evidence-based care and monitor patient profiles daily.
• Drive improved outcomes: reduce 30-day readmissions and hospital dissatisfaction indicators.
• Adhere to organizational competencies and standards of behavior.
