Job summary

Hackensack Meridian Health is seeking a Unit Based Pharmacist to join our patient-centered medication management team in a hospital setting.

The role integrates clinical and distributive pharmacy activities with physicians and the interdisciplinary team to improve outcomes. New Jersey license is required with PGY1 completion and ACLS within 6 months. A Bachelor of Pharmacy is required, with Doctoral degree preferred.

Qualifications

• Pharmacy degree from an accredited college.

Responsibilities

• Validate medication reconciliation on admission and prior to discharge.
• Perform patient assessment and counseling on medications and regimens.
• Identify barriers to adherence and facilitate safe discharges.
• Collaborate with doctors to optimize cost-effective, evidence-based therapy.
• Monitor patient medication profiles and participate in rounds.
• Adhere to organizational competencies and standards of behavior.
• Handle other duties and projects as assigned.
