Description

Coordinates care for Pacific Medical Centers' Managed Care patients to ensure cost-effective and appropriate utilization of healthcare resources. The UM Nurse Case Manager performs utilization management of services and coordinates the entire spectrum of care for patients across various settings, both inpatient and outpatient. This position supports physicians (internal and external) with care management and coordination. It is a hybrid role, combining remote and in-person work.
