Job summary

FCC Behavioral Health is seeking an UBHCC Nurse to support a multidisciplinary crisis team at the Urgent Behavioral Health Crisis Center in Poplar Bluff, MO. The role involves vital signs, triage, and coordinating care pathways to ensure timely engagement with next providers.

The Nurse will conduct basic nursing assessments, monitor patients’ health status, and document thoroughly while assisting with telehealth connections for NP evaluations and medication management.

Qualifications

• Must be a licensed LPN (or RN).
• Experience in psychiatric or behavioral health care and healthcare technology is preferred.
• Ability to conduct basic nursing assessments.

Responsibilities

• Obtain and record vital signs.
• Perform blood and urine collection as requested by prescribers.
• Reconcile home medications.
• Complete triage assessments.
• Determine appropriate care pathways from triage outcomes and coordinate with the next provider.
• Monitor clients’ physical and behavioral health status during visits.
• Communicate information to the clinical team.
• Conduct violence risk screening using the agency tool.
• Review PDMP information.
• Facilitate telehealth connections for NP evaluations.
• Call in prescriptions and refill medications as directed by prescribers.
• Complete documentation in a timely manner.
• Communicate information clearly orally.
• Adhere to CARF/DMH standards and agency policy.
• Participate in special projects and agency initiatives.

• Communicate information orally in an effective and accurate manner.
• Assure program operates with the constraints of the agency’s certification (CARF, DMH) standards and strategic plan related to clinical care.
• Abide by program and agency policies and procedures.
• Other Duties: Participate in special projects and agency initiatives as assigned.
