What to Expect (Job Responsibilities):
  • Conduct comprehensive pre-admission screenings (PAS) for customers applying for Arizona Long Term Care Services (ALTCS)
  • Manage and monitor PAS caseload, ensuring timely completion of assessments and documentation
  • Obtain and review medical records and relevant data to support eligibility findings
  • Collaborate with other state agencies and community resources to provide necessary support to customers
  • Attend staff meetings and contribute to the exchange of professional information in support of the program
What is Required (Qualifications):
  • Minimum of 3 years of experience in social service case management or a related field; OR graduation from an accredited nursing school with an active Arizona RN license; OR a Bachelor's or Master’s degree in a related field with 2 years of relevant experience
  • Knowledge of medical, functional, and psychosocial issues affecting elderly and disabled populations
  • Effective communication skills, both orally and in writing
  • Ability to manage a caseload and maintain timely documentation
  • Valid Arizona Driver License
How to Stand Out (Preferred Qualifications):
  • Bilingual (fluent) in Navajo for specific office locations
  • Bilingual (Spanish) is a plus but not required for all offices
