What to Expect (Job Responsibilities):
- Conduct comprehensive pre-admission screenings (PAS) in both institutional and community settings
- Manage and monitor PAS caseload, timeliness, and documentation using HEAplus
- Obtain and review medical records and interact with family members and professionals to evaluate eligibility
- Communicate assessment outcomes to applicants and provide community resources
- Attend staff meetings and collaborate on special projects and financial eligibility interviews

What is Required (Qualifications):
- Minimum 3 years of experience providing social service case management or relevant education and experience combination
- Knowledge of medical, functional, and psycho-social problems of various populations
- Ability to effectively communicate both orally and in writing
- Proficiency in using a personal computer for documentation and case management
- Valid Arizona Driver License

How to Stand Out (Preferred Qualifications):
- Bilingual (Spanish) is a plus but not required
- Experience in healthcare delivery systems and community resources
- Familiarity with State and Federal rules concerning long-term care and medical services
- Experience working with elderly, physically disabled, and developmentally disabled populations
