Washington County School District seeks a Transition Case Manager to oversee the Transition School to Work (TSW) program. You will coordinate referrals, ensure VR eligibility documentation is completed, and work with LEA staff and VR counselors to align services with IPE and IEP goals.

The role requires a B.S./B.A. in education or human service fields and at least two years of related experience with individuals with disabilities.

Qualifications

• B.S./B.A. in education or human service related fields.
• Two years' experience in teaching or services to individuals with disabilities.

Responsabilités

• Receive referrals from LEA and inform VR Counselor; assist VR Counselor in completing application.
• Maintain confidential student records and ensure privacy.
• Coordinate meetings with referring VR Counselor and LEA.
• Schedule and attend events and trainings with VR Counselor and other TSW staff.
• Attend IEP Team Meetings and report student progress.
• Document student progress and prepare documentation for future employment.
