Responsibilities

• Assists with personal care activities
• Assists with household tasks
• Meets safety needs of clients including safe use of equipment
• Accurately documents services provided in accordance with agency policies
• Participates in the supervisory process with other health care team members
• Performs other duties as assigned by supervising nurse
• Reports changes in client condition to RN supervisor
• Maintains client privacy and records
• Adheres to dress code and maintains hygiene
