Qualifications:

• Current and unrestricted RN license in Massachusetts Current BLS CPR certification
• Minimum of one year of nursing experience, preferably in ER or outpatient triage setting.

Principal Responsibilities and Duties:

• Receives and responds to both telephone and in-person triage requests from patients.
• Conducts nursing assessments, exercises clinical judgment, answers health-related questions, provides patient education, and offers guidance following established clinical protocols and guidelines.
• Documents telephone triage, patient education, and patient instruction accurately and precisely within the electronic medical record.
• Schedules same-day and follow-up appointments with providers to support timely and coordinated patient care.
• Provides ongoing patient education, clinical assessment, and support to patients while working closely with medical providers to deliver coordinated, high-quality nursing care.
• Performs nurse visits as needed, providing clinical assessments, patient education, and nursing interventions following established protocols.
• Administers prescribed medications, vaccines, and treatments following approved clinical protocols and standards of nursing practice.
• Ensures accurate and timely documentation of all administrations in the electronic medical record.
• Serves as a resource for staff, offering clinical guidance, nursing support, and educational opportunities as appropriate to promote staff development and quality patient care.
• Other duties as assigned.

Working Conditions:

This position involves prolonged use of a telephone and extended periods of computer-based work at a workstation. It requires the ability to sit for long durations, with occasional standing and walking May be required to lift and carry objects typically weighing between 5-20 pounds.
