Job summary
Passavant Memorial Homes in Rochester, PA is hiring a full-time Triage Nurse to support individuals with intellectual disabilities and behavioral health needs. The nurse will perform selected nursing acts, monitor on-site health needs, and coordinate with hospital staff while reporting to the Clinical Services Director and Regional Program Directors.
The role requires PA nursing licensure, a valid PA driver's license, and 1–2 years of medical/surgical or geriatric nursing experience.
Qualifications
- Graduate of an accredited school of professional and/or practical nursing with a PA license by examination.
- Valid PA Driver’s License required.
- Understanding of the Intellectual Disabilities field preferred.
- 1–2 years’ experience in medical/surgical or geriatric nursing preferred.
Responsibilities
- Perform clinical nursing skills with fundamentals knowledge.
- On-site monitoring of community residential settings as required.
- Monitor staff completion of new medication orders and documentation.
- Provide ongoing health assessments and care planning.
- Administer injections as needed (e.g., Depo-Provera, Hepatitis B, TDap).
- Complete dietary in-services for specialized diets and forward to homes.
- File labs, physician orders, consults and related paperwork.
- Communicate daily with hospital staff and update Clinical Services Director.
