Primary Health Solutions in Hamilton, OH is seeking a Triage LPN for our Administration office. This full-time role offers Mon-Fri 8:30am-5:00pm and involves patient triage, education, scheduling, and care coordination to support providers and patients.

You will handle multi-line calls, document in charts, assist with drug program education, and ensure excellent customer service while upholding compliance and quality standards.

Qualifications

• Graduation from an accredited LPN program.
• Must hold a current, valid LPN license in Ohio.
• Ability to read and interpret safety rules, operating instructions, and procedures.
• Ability to write routine reports and correspondence.
• Ability to communicate effectively before groups of customers or employees.

Responsibilities

• Assists patients with routine questions, resolves minor difficulties, and directs patients as authorized by the Provider; documents communications in patient charts.
• Supports patient education and assistance of drug programs; provides education and support to patients and families.
• Schedules appointments as needed and coordinates care following hospital or specialty follow-ups.
• Advices patients on payment options and aids in understanding insurance eligibility and assistance programs.
• Maintains accurate records and assists with care coordination and follow-up.
