Qualifications
  • Graduate of an approved Licensed Practical/Vocational Nurse program and licensed in the state of practice, required.
  • One-year experience providing specialized treatments in long term care setting.
  • Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred.
  • Current BLS/CPR certification (All SNFs; ALFs as required by state).
Responsibilities
  • Perform resident skin assessments, obtain treatment orders, administer treatment as ordered, and document appropriately to ensure that resident skin integrity is improved and/or maintained.
  • Document findings appropriately and contact physicians for treatment updates; monitor wound healing progress and suggest changes as needed.
  • Maintain records of pressure ulcers and skin alterations, including risk assessments and prevention activities.
  • Investigate and implement center practices and risk minimizing measures, including accurate completion of Braden risk scales on admission.
  • Monitor timely implementation of preventive interventions for at-risk residents.
  • Educate new nursing staff on skin care protocols and resources.
  • Ensure availability of wound care supplies and coordinate with central supply.
  • Assist with case reviews for pressure ulcers.
  • Stay updated with regulations and industry practices; pursue ongoing education in skin and wound management.
  • Provide reports and recommendations for quality improvement.
  • Perform other duties as assigned.
