Job description

AHRC NYC is looking for a Full-Time Treatment Coordinator/QIDP for our Agovino ICF residence in the Bronx.

The Treatment Coordinator/QIDP is responsible for coordinating the training and habilitation program of the assigned residents with residential staff, educational and vocational rehabilitation programs, and other needed services.

Salary: $55,000-$65,000 per year plus a comprehensive Benefits package...see Benefits information below.

Location: Bronx

Schedule: 8 hour Day shift Monday thru Friday

ESSENTIAL RESPONSIBILITIES
  1. Serve as a member of the interdisciplinary team to coordinate the integration of various services in planning, implementation, and evaluation of each individual’s treatment plan.
  2. Generate Needs/Capabilities list annually, with updates as needed. Ensure each resident has:
  • An activity schedule providing an hourly account of the resident’s day, 24/7, updated regularly.
  • Ongoing active treatment.
  • Implementation of the activity schedule as documented.
  • Act as a resident advocate in areas such as cleanliness, clothing, personalized and clean bedrooms, toiletries, etc.
  • Review each resident’s individual treatment plan regularly, summarizing progress and recommending modifications based on concrete, measurable data obtained by direct care staff.
  • Coordinate with day/workshop programs through ongoing face-to-face and phone contact, ensuring service coordination between day and residential components.
  • Ensure timely receipt and proper filing of IPP’s and day program progress notes.
  • Train direct care/clinical staff in behavioral interventions and active treatment areas to ensure proper implementation of treatment plans.
  • Coordinate the Annual Review process by:
    • Ensuring collection of reports from all disciplines in a timely manner.
    • Preparing annual reports and updates on assessments, medical, nutritional, recreational, sensorimotor, communication, skills, behavior, incident reports, previous treatment review, and financial status.
    • Documenting rationale for interventions based on identified needs for the upcoming year.
    • Generating goals, objectives, and interventions for indicated needs.
